Health Protection Scotland (HPS) would like to request that all Scottish designated Yellow Fever Vaccination Centres (YFVC) now submit their annual, yellow fever vaccine utilisation figures for 2014.
Preferably returns should be submitted electronically, however, they can be mailed or faxed to HPS. A PDF of the vaccine returns form for the period 1Jan to 31 Dec 2014 can be downloaded. Annual returns should be submitted by 1 March 2015.

It is expected that details of the required information for this return will be posted on the NaTHNaC website in early January 2015, together with a link to the AR survey. This survey should be submitted electronically to NaTHNaC within a specified time frame. YFVC in EWNI will also be notified by email when this link is open.
